To apply for financial support from Pacifica Family Fund, please complete the following steps:
Step One:
Sign up for an Orientation at Pacifica Family Maternity Center. Orientations are held every other week and are currently virtual due to the coronavirus pandemic. If you are unable to make any of the dates/times listed, please email midwives@pacificamaternity.com and we can accommodate your schedule!
Step Two:
One you’ve attended the orientation you can email pacificafamilyfund@gmail.com to receive a digital application. The application form has a Financial Assistance Eligibility Table (see below also), so you can see what level of support may be available for your family, based on your annual household income.
Step Three:
Submit your application to pacificafamilyfund@gmail.com
When we have received your application, we will send you notice of receipt within 1 week. You will then receive a formal eligibility decision within the next few weeks.
